Provide outreach and engagement activities concerning active and potential consumers, provide comprehensive assessment and service planning with consumers who have chronic medical problems or significant mental illness, able to manage caseloads that vary from 15-60 persons, provide care management activities that include face-to-face meetings, mailings, electronic communications and telephone calls with consumers, their families and various community service providers. MINIMUM QUALIFICATIONS: Bachelor’s degree in Human Services related fields plus two(2) years of experience providing care to persons with mental illness, chemical dependency, developmental disabilities and/or chronic medical condition OR High School Diploma plus five(5) years of experience providing care to persons with mental illness, chemical dependency, developmental disabilities and/or chronic medical condition. Candidate must have a car, valid NYS driver’s license and pass background check.
