Your care manager is ready to assist YOU.
A Care Manager ensures that everyone involved with an individual’s care is working together and sharing information that is important in the promotion of wellness.
From the start, each person is assigned a Care Manager. A BFNC Care Manager will work with you to discuss what your wellness goals are. Care Managers assist in the following areas:
- Housing
- Food
- Transportation Assistance for medical appointments
- Linkage to health services to assist with reducing Emergency Room visits and hospitalizations
Care Management services include:
Health Home Care Management >
Provides free comprehensive care management services to individuals with Medicaid insurance, to assist in the coordination of services to address medical or mental health needs.

Assisted Outpatient Treatment (AOT) Care Management >
Services are provided to individuals with a mental illness who, because of their treatment history and present circumstances, are unlikely to survive safely in the community without supervision.
Critical Time Intervention (CTI) Care Management >
For Individuals 18 years and over living with a serious mental illness.
Non-Medicaid Care Management >
For Individuals 18 years and over living with a serious mental illness who do not have Medicaid.
