    Your care manager is ready to assist YOU.

    We provide free, comprehensive Care Management services to individuals with Medicaid insurance, to assist in the coordination of services to address medical or mental health needs.

    Who is eligible for health home care management?

    • Individuals age 18 years and older
    • Living in Erie County
    • Already members of a Managed Care Organization
    • AND have Medicaid.

    AND

    • Have a diagnosis of two or more chronic health conditions such as asthma, diabetes, heart disease, etc.
    • OR a mental health diagnosis
    • OR Living with HIV/AIDS.
